Why do these underground collissions continue? Is it lack of training or operators simply not paying attention? Have the investigations uncovered any mechankical issues? And finally, has the NTSB conducted an investigation into any of the accidents?

It's operator error contributed by Septa's poor lighting of the tunnel & also making nightliners 10+ hrs. The tunnel is not operator friendly, and also other operator's go down there & drag the crap causing the operator behind you to start 2 suffer from fatigue. REMEMBER WE DON'T STEER IT'S EASY 2 FALL ASLEEP DOWN HERE EVEN WITH THE PROPER REST.
