Ft. Washington Exp [on route signs. it was simply marked "EXP"] = became the 4 then abandoned shortly after SEPTA created the 201.

Horsham Breeze = 310 [and abandoned 311]
The 4 actually held on until the mid-1990s (between 1995-1997; don't remember the exact year offhand).

311 = Commonwealth Breeze

Don't forget Shoppers Specials 10 and 11 that came along fairly late in the game.

There were also many Race Track Specials (to Liberty Bell Park, where Franklin Mills now stands) that to the best of my knowledge never carried numbers.
One of the Race Track Specials, IIRC, operated from 69th Street. Where did the other ones operate from?

I beleive the 4(Broad & Olney Avenue to Fort Washington) became extinct in either June or September 1996.

As I recall, Race Track Specials ran from Frankford Terminal (express to the track) and Olney Terminal (also, I believe, express to the track), as well as starting at Broad & Oregon and making stops on Broad St at Snyder Ave and in Center City. A few years ago someone told me there was another that started at Ridge & Green Lane and made some stops on Ridge Ave before heading up the Boulevard to the track. I seem to recall one from the Germantown/Chelten area as well.
