My reasonably serious injuries, from lifting only:

Dislocated both shoulders and partially tore both biceps returning a barbell to the rack after doing heavyish (260kg) shrugs - in my young stupidity tried to return the bar to the rack with a flourish, one side either missed or bounced out of the saddle and, being strapped to the bar, there was no way for my to let go.
Anyway, that injury basically spelled the end of serious lifting for me for five years or so, and even today gives me huge problems, I have to literally tip-toe around it - I've probably re-aggravated those injuries ten to fifteen times in the last five years or so, pressing, curling, pull-ups etc are by and large rare beasts in my training these days. Still, the good thing is that I can tell with some certainty now if it is going to snow.

Severely strained tendons in both hands doing finger lifts, for a while I thought I had lost use of my left hand as anything other than a dibber. Still have problems with grip work these days.

Of course, out side of that, I've done stupid things like dropped barbells on my head in the quick lifts and dropped plates on my feet etc etc, probablymore times than I can count.
